Root Cause Analysis in DMM

DMM provides extensive traceability for all types of inspections, condition assessments, and incidents. Among other things, DMM includes flagging of events or records that indicate an abnormal condition or that something has gone wrong. These flags can then be placed into the appropriate resolution process within DMM. Furthermore, that resolution can include any kind of documentation, and in addition, DMM now offers a standardized root cause analysis in the form of the “5 Whys,” which is easy to link to flags involving serious deviations.

Example of a 5 Whys root cause analysis might look like this:

Problem: There is water on the floor

Why 1: A pipe in the ceiling is leaking

Why 2: The pressure is too high

Why 3: The control valve is not functioning properly

Why 4: Control valves have not been tested

Why 5: Regular testing procedures are not in place
